continues to grow in its mission to support individuals with a variety of developmental disabilities, mental illness, traumatic brain injuries, physical challenges, and other medical needs. The Paragon Activity Club (PAC) is a respite program providing growth and opportunity for children 8-14 yrs and adolescents 15-21 yrs through creative programming in a recreational day service setting, as well as in our community. We are currently seeking a PAC Team Lead.

RESPONSIBILITIES include but are not limited to: quality assurance of daily operations, new program development, progress reviews; direct supervision and scheduling of the team; development, presentation, and monitoring of the Individual Support Plans with the coordination of their Support Team at the times prescribed by policy, procedure, contract and/or regulation; ability to identify and teach Program Instructors to use teachable moments to facilitate meaningful experiences; team support in providing individuals opportunities to participate in a diverse variety of daily activities; manage multiple projects with strong organizational skills; ensure all program activities, schedules, community events, off grounds transportation, feeding, medical, therapy, and other needs occur as defined; coordination of events, meetings, and tours internally as well as with case managers, residential providers, guardians and potential customers; Community out reach to make contacts including, but not limited to, school districts, group home providers, businesses, public institutions, and advocacy groups; administrative as well as Service related record keeping, documentation and general correspondence; direct / personal care support as needed; providing participant / staff transportation in agency vehicles; demonstrate a proactive approach to the health and wellbeing of all participants; ensuring supervision and safety of participants in a safe and clean working environment.

QUALIFICATIONS include being available for rotating Saturdays; available for scheduled / "day of" call-in coverage and flexible 1st/2nd shift administrative or supervisory hours; min. 2 years post-high school education in Human Services, Vocational Rehabilitation, or related field - Bachelor's degree preferred; 2 yrs supervisory experience preferred; experience supporting children/adolescents/ adults with special care needs / challenging behavioral supports; being responsible, committed, competent, self motivated with a positive attitude while working as an appropriate social role modeling to the team and participants; excellent written / verbal communication with stakeholders; computer skills including experience with Microsoft Office Suite.  Must be able to travel between both Oconomowoc and New Berlin sites to provide appropriate coverage.  Must have a valid WI driver’s license with 3 years driving experience an acceptable driving record. Must pass a pre-employment physical and TB screen. Must participate in a drug screening program. Must have an acceptable criminal background check. EOE
